Requests for copies of Police Reports must be made in writing. For your convenience, a Report Request Form is available here:
Please complete the report request form and provide a copy of a valid form of identification. Your payment must also be submitted at the time of your request. Please refer to the Report Request Form or below for pricing. You may contact the Records Unit telephonically for price confirmation. Please allow three to five working days for UCPD to process your request. Requests may be made in person or mailed to:

All other requesters must petition the Presiding Judge of the Juvenile Court directly for the release of juvenile records. The Juvenile Court has the exclusive authority to determine whom, and the extent to which juvenile record information may be released. The required forms are: Petition to Obtain Report of Law Enforcement Agency (JV-575) and Notice to Child and Parent/Guardian re: Release of Juvenile Police Records and Objection (JV-580). The completed forms can be mailed or delivered to the Los Angeles County Superior Court. After service of the required documents on all interested parties, and the time to object has expired, the Presiding Judge of Juvenile Court will rule on the request.
The cost of a report is listed below with the exception of a Domestic Violence report which is released at no cost to the victim pursuant to Family Code section 6228. If your request is approved, you will be contacted to arrange pick-up and payment of the report. If your request is denied, you will receive a written denial in accordance with Government Code section 6255(b).
The cost to obtain a police or EMS incident report is $17.00.
The cost to obtain a police arrest report is $25.00.
Traffic accident reports have a separate fee structure:

Checks should be made payable to “U.C. Regents”. Major credit cards are accepted in person at the UCLA Police Department. Cash is NOT accepted.
Background Clearance Letters are provided to requestors who reside in the immediate vicinity of the UCLA Police Department and campus community. The fee is $25.00 and takes approximately three to five business days to process.
